Charge Capture Review

Post-Payment Review
According to AMA's 2012 National Health Insurer Report Card the payments made from commercial payers only matched the contracted fee schedule 91% of the time.

Denials Review
According to AMA's 2012 National Health Insurer Report Card the average percentage of all claim lines that are denied because of lack of information is 21%. Some commercial payers reported up to 40% of all denials are because of lack of information.
